Minimum Data Set (MDS)
Certified Medicare or Medicaid nursing facilities are required to complete and submit, electronically, the Minimum Data Set (MDS). This assessement tool must be completed for all residents, regardless of age.
Minimum Data Set - A core set of screening, clinical, and functional status elements, including common definitions and coding categories, which forms the foundation of a comprehensive assessment for all residents of nursing homes certified to participate in Medicare or Medicaid.
While its primary purpose as an assessment tool is used to identify resident care problems that are addressed in an individualized care plan, data collected from MDS assessments is also used for the Medicare reimbursement system, Louisiana's State Medicaid reimbursement system, and monitoring the quality of care provided to nursing home residents.

MDS & OASIS submission process changes effective 12/27/11 for LA. LA providers must convert from AT&T to Verizon within 30 days of the effective date. Details on the CMS QTSO Provider Information website.
