This instructions and forms in this packet are to assist providers in changing the entity name of their hospital, with or without a corresponding change in the DBA (doing business as) name of the hospital. If only the DBA name is changing, please use the Hospital DBA Name Change Packet instead of this one. If the entity name change is a result of a change of ownership (CHOW), please use the Hospital CHOW Packet instead of this one.
