Assessment & Care Planning Trainings Schedule

 

 

MDS-HC and Care Planning Training

The MDS-HC and Care Planning Training is offered only to Louisiana Support Coordinators/Assessors who work for a Support Coordination Agency, PACE, or Xerox staff that provide services to recipients on the Community Choice Waiver, Adult Day Health Care Waiver, PACE, and/or LT-PCS.

New support coordination agencies will be informed through the enrollment process by Office of Aging and Adult Services when to register for the training.

The MDS-HC training consists of two classroom days, the completion of two field assessments  approved by the trainer, and a comprehensive on-line test. Care Planning Training requires two classroom days, completion of two Plans of Care approved by the trainer, and a comprehensive on-line test. Once a support coordinator has passed both trainings, they will receive MDS-HC and Care Planning certification which is good for three years through Department Health and Hospitals - Office of Aging and Adult Services.

Re-certification Training

The Re-certification Training is offered only to Louisiana Support Coordinators/Assessors who are certified assessors and work for a Support Coordination Agency, PACE, or Xerox staff that provide services to recipients on the Community Choice Waiver, Adult Day Health Care Waiver, PACE, and/or LT-PCS.

The Re-certification training consists of two classroom days enhancing the assessor’s skills of MDS-HC Assessment and Care Planning. Once a support coordinator has passed a comprehensive test, they will receive MDS-HC and Care Planning re-certification which is good for additional three years through Department Health and Hospital- Office of Aging and Adult Services.
