Assessment & Care Planning Trainings Schedule
The MDS-HC and Care Planning Training is a training offered only to Louisiana Support Coordinators/Assessors who work for a Support Coordination Agency, PACE, or Xerox staff who provide services to recipients on the Community Choice Waiver, Adult Day Health Care Waiver, PACE, and/or LT-PCS.
New support coordination agencies will be informed through the enrollment process by Office of Aging and Adult Services when to register for the training.
The MDS-HC training consists of two classroom days; completion of two field assessments, approved by the trainer; and a comprehensive on-line test. Care Planning Training requires two classroom days; completion of two Plans of Care, approved by the trainer; and a comprehensive on-line test. Once a support coordinator has passed both trainings, they will receive MDS-HC and Care Planning certification which is good for three years through Department Health and Hospital- Office of Aging and Adult Services.
