About the Program

History

The Louisiana Diabetes Control Program (DPCP) is funded through a cooperative agreement from the Centers for Disease Control and Prevention (CDC), National Center for Chronic Disease Prevention and Health Promotion. The Louisiana DPCP began receiving funding from the CDC in October 1996. The Louisiana DPCP is housed within the Louisiana Department of Health and Hospitals' Office of Public Health, Bureau of Primary Care and Rural Health - Chronic Disease Unit.

Mission

The mission of the Louisiana Diabetes Prevention and Control Program (DPCP) is to reduce the morbidity and mortality of diabetes in Louisiana.

We strive to achieve this mission by providing diabetes-related technical assistance and support to individuals, health care providers and agencies; maintaining a reliable diabetes surveillance system and eliminating gaps in Louisiana's diabetes public health system.

  • Goal 2: Prevent complications, disabilities and burden associated with diabetes
  • Goal 3: Eliminate diabetes-related health disparities
  • Goal 4: Maximize organizational capacity to achieve the National Diabetes Program goals.

Purpose

The Louisiana DPCP is an affiliate of the Centers for Disease Control and Prevention's Systems Based Diabetes Prevention and Control Programs Initiative. CDC funds statewide diabetes programs in all 59 states and territories to address the "Healthy People 2010" focus areas of Diabetes, Immunizations, Access to Quality Health Services, Chronic Kidney Disease, Heart Disease and Stroke, Vision and Hearing, Nutrition and Overweight, Physical Activity and Fitness, and Public Health Infrastructure.

Measurable program outcomes derived from this initiative must be in accordance with the National Center for Chronic Disease Prevention and Health Promotion's (NCCDPHP) performance goal to "Increase the capacity of state diabetes control programs to address the prevention of diabetes and its complications at the community level."

Intervention strategies

  • Establish and maintain a presence in the health department for diabetes prevention and control
  • Define the burden of diabetes in the state and communicate it in a manner that informs and influences public health decision making
  • Provide ongoing continuing education and technical support to Louisiana's diabetes health care and public health workforce
  • Establish and maintain effective partnerships
  • Develop a State Diabetes Strategic Plan
  • Work to eliminate gaps in Louisiana's diabetes public health system through information sharing, funds development and establishing new services
  • Engage in small scale pilots

Whenever possible, the program will engage in and support efforts involving pre-diabetes, diabetes prevention among adults and children and other related areas.
