How can my child get medical care from CSHS?

A child can get help from CSHS if he or she:

• Lives in Louisiana
• Is less than 21 years old
• Has a health condition that CSHS can treat -- Click here to download the CSHS brochure so you can find out what health conditions CSHS treats
• Has Medicaid or meets CSHS income rules -- Click here to find the contact information for your regional CSHS clinic staff so you can get more information on the specific income rules

How do I apply?

You can apply for services in 1 of 3 ways:

If you want to mail the application to CSHS

1) Download a CSHS Application (click here).  Find the CSHS office that is near where you live; click here to find your nearby office. Next, mail the filled out application to your nearby CSHS office.  Please note: So that the application process goes smoothly, please be sure you have included one of the following 3 things when you mail in the application:

• Please have your child's medical provider (doctor, nurse practitioner, audiologist) fill out and sign Section 5 (Diagnosis Confirmation) of the application; 

OR

• Please have your child's medical provider (doctor, nurse practitioner, audiologist) give you a note with your child's diagnosis written on it;

 OR

• Please have a copy of your child's medical records that contains information about your child's diagnosis.

OR--If you want to fill out the application with someone at a health unit:

2) Call the parish health unit in the parish you live in, and make an appointment to fill out the CSHS Application. If you do not know where your local parish health unit is, click here.  Please note:  You will need to bring to the appointment your child's medical information that describes your child's diagnosis.  Please bring either a note from your child's medical provider (doctor, nurse practitioner, audiologist) or a copy of your child's medical records.

OR--If you want to fill out the application with someone at CSHS:

3) Call the CSHS regional office near where you live to make an appointment to fill out the CSHS Application; click here to find your nearby CSHS office. Please note:  You will need to bring to the appointment your child's medical information that describes your child's diagnosis. Please bring either a note from your child's medical provider (doctor, nurse practitioner, audiologist) or a copy of your child's medical records.
